The Disability Determination Service (DDS) makes the decision as to whether or not you’re disable based on your medical record.  To receive a copy of your medical record to provide with your application, you may be asked by your health care provider to pay up to $0.75 a page and a search fee of up to $10.

If you’re not able to pay for the costs of obtaining your medical record, you may be able to request a free copy by providing financial proof of your inability to pay the fee.  You’ll also be asked to give your doctor’s office a Release of Information form so they can pass the information on to the Social Security Administration.

How Much Will You Receive From Federal and State

The city of Huntington, as well as other cities in West Virginia, do not receive a state supplement for SSI.  They only receive the federal base amount which is $733 for individuals and $1,100 for married couples.  If you receive income from other sources including from family and friends as well as food stamps, it is subtracted from the amount you receive from the Social Security Administration.
